The process for request of a matching grant from the Masonic Medical Foundation of Wisconsin, Inc., for a Project A.D.A.M. defibrillator, for a Wisconsin High School, requires three steps. Completion of these steps is necessary in order to move your grant to approval as quickly as possible.
- Define the program or project – This can be done in a letter addressed to the Board of Directors of the Masonic Medical Foundation of Wisconsin, Inc. It should include what high school you would like to support; the expected cost; and a brief description of what the program is and whom it serves.
- Prove financial stability of your recipient – Obtain the high school's most recent annual budget (usually available in the guidance or principal's office). This usually provides a good review of their financial condition. Most schools have this prepared and on hand for the purpose of securing grants from foundations like ours.
- Include a check for your lodge’s share of the match – This ensures prompt processing of your request once the foundation board has approved it. The check should be made payable to the Masonic Medical Foundation of Wisconsin.
All Project A.D.A.M. Grants have been pre-approved by the Foundation's Board of Directors. Therefore, once we have received all of the necessary paperwork, we are able to return a check for the entire project. For Project A.D.A.M. requests only, it is not necessary to wait for the Board of Directors to vote upon it at a quarterly meeting, the grants are pre-approved.
